Sustainable National Health Insurance and Social Solidarity
一個永續的福利體系，以社會團結為基礎，不論遭遇各種內外在壓力，仍持續穩定確保未來世代能與現在世代有約略相同的機會，公平享有體系所保障的那種資源或權利。全民健康保險作為臺灣福利體系之一部，目的是保障人們因醫療需要所生財務負擔。全民健康保險因其隨收隨付之財務設計，加上臺灣人口與經濟結構條件，必須不斷預支未來資源用以滿足現在世代的醫療需要，此為全民健康保險永續問題的根本構造。未來改革可加強重分配，並反思構成全民健康保險的社會團結。
A sustainable welfare system is underpinned by social solidarity, maintaining the equal opportunity for the present and future generations to equitably enjoy the resources or rights that the system aims to ensure. The National Health Insurance (NHI), as an integral part of the welfare system in Taiwan, aims to protect people from the financial burdens due to healthcare needs. Its pay-as-you-go financial basis, along with the population and economic structure in Taiwan, has compelled the intergenerational transfer of resources from the future to meet the needs of the present generation. This is the origin of the sustainability issue of the NHI. Reformers could consider more redistributive financial arrangements, and should reexamine the solidarity that is supposed to underpin the NHI.
