我國過去所係採論量計酬之健保給付制度，亦即每一項照護服務都能帶來收入；然而，論量計酬卻引發了過度醫療的問題，除不必要的成本支出、醫療資源無法合理分配，對病患也增加了非必要的檢查與住院時間。為考量成本控制與品質提升，我國已分階段導入住院診斷關聯群Tw-DRGs項目，就同一種疾病，依年齡、處置、併發症以及住院與否等條件分類，採同病同酬的支付方式。此時，醫院必須主動考量成本支出，而過度醫療所滋生的問題也將因此避免。
The Health Insurance Payment depended on the amount of outpatient. Each service brings income. However, depending on the amount of outpatient caused the problem of the excessive medical service. Besides of unnecessary costs and unreasonable distributions of medical resources, the unnecessary examinations and hospitalizing of the patient were increased. In order to control the cost and to enhance the quality, Taiwan Diagnosis related Groups was introduced step by step. It classifies the illness according to the ages, handling, complications and whether the patient would be hospitalized. Its payment depends on the kinds of illness. Therefore, the hospital must take the cost into consideration and the problem of the excessive medical service would be solved as the result.
